Café Central 

I believe this to be Madrid's most venerable surviving jazz venue. Whether or not I am right, the Café Central programs top quality names within the sort of price bracket a smaller-than-medium-sized cafeteria can afford, and achieves what is to my taste the most authentic, intimate jazz ambience you can find in Spain. If you're a jazz enthusiast visiting Madrid, this is the most unmissable place in town.

Café Populart 

Very small and very central music bar which puts on very high-class acts and which unsurprisingly gets very crowded.

Calle 54 

Restaurant in Calle Habana in the north of Madrid, named after the very fine documentary film by Fernando Trueba released in 2000 and dedicated to Latin jazz (hence, presumably, the choice of location for the restaurant). If jazz chic or overpriced cocina nueva is your thing, I expect you'll want to visit it. Wouldn't be seen dead there, myself.

Clamores 

One of the most veteran jazz spots in Madrid, the Sala Clamores puts on concerts by top Spanish and foreign jazz artistes and performers in other styles, names like Jerry Goodman, Joe Henderson, Johnny Griffith, Lou Bennet, Terence Blanchard, Tete Montoliu, Pedro Iturralde, Jorge Pardo, Chano Domínguez, Andy Summers, Danilo Pérez, Sonny Fortune, Compay Segundo, Susana Rinaldi, Labordeta or the Vieja Trova Santiaguera, in a relaxed, pleasingly smoky atmosphere.
