In a word, where's good? Taking the ferry to Bilbao, so will have the car. Is Bilbao worth hanging around in or do we just head on out? And in which direction?

You're spoilt for choice, especially with wheels. You don't say how long you've got or when you're going, but you could easily spend a month doing the Basque Country alone. In that direction, westward, you've got Navarre as well, then the Pyrenees, Aragón... Or you could head west through Cantabria and Asturias, maybe as far as Galicia. You're looking at the best of less-known Spain. And Bilbao is definitely worth a couple of days, in itself - you'll need a good half day just for the Guggenheim.
